Personal History Forms
A Personal History Form (see below), the appropriate Authorization and Release Form, and the Background Check Consent Form must be completed by all who wish to be considered for appointment to the federal judiciary. All forms must be signed, dated and sent by mail, together with any additional supporting material to:
